Sick Visit: $185

*Includes exam, treatment, and up to 3 in-house testing.

Procedure: $245

*Includes exam, treatment for: laceration repair, abscess drainage, foreign body removal and etc.

New Patient Wellness Visit: $255

*Includes exam, treatment plan, and in-house testing (EKG, urinalysis, and blood sugar test).

Establish Patient Wellness Visit: $185

*Includes exam, treatment plan, and in-house testing (EKG, urinalysis, and blood sugar test).

Confidential STD Testing Visit Level 1: $365

*Includes exam, treatment, in-house medication administration- injection, urine testing for Chlamydia, Gonorrhea, and Trichomonas.

Confidential STD Testing Visit Level 2: $460

*Includes exam, treatment, in-house medication administration- injection, urine testing (Chlamydia, Gonorrhea and Trichomonas), blood testing (HIV, Syphilis, and Hepatitis), and/or lesion sample testing.

DOT Physical: $110

*Durable medical equipment (such as splints, braces, orthopedic boots, and crutches) are offered at an additional cost (prices may vary).

*Vaccinations, immunizations, and physicals (suchas school/sports/camp and pre-employment, etc.) are priced individually.

*Imaging and lab tests sent to outside laboratory are priced separately (excluded from wellness and/or sick visit consultations).
